HomeHero, Cedars-Sinai Link On Patient Transition Program

Santa Monica-based HomeHero, the home care technology and services startup founded by Kyle Hill and Mike Townsend, has linked up with Cedars-Sinai to launch a new program called Safe Transition Home. According to HomeHero and Cedars-Sinai, the new program will help transition patients from the hospital to home, with the goal of reducing inpatient readmissions, raising patient satisfaction and improving health outcomes. HomeHero, which is backed by Graham Holdings, Social+Capital Partnership, Science Inc, The Launch Fund, Tencent Holdings, Techstars and Cedars-Sinai Medical Center, is currently part of the 2016 Techstars Healthcare Accelerator being run at Cedars-Sinai. Financial impact of the deal was not announced.
